ACCEPTABLE ELECTRONIC FUND TRANSFER (EFT) ACCOMPANYING DOCUMENTS - 2021
VOIDED CHECK
- Account holder’s information MUST be printed on the check and MATCH the name of the agent being contracted [NOTE: Should ultimately correspond with the legal name printed on their license].
- If the agent is being contracted as an individual, then the voided check MUST have the individual’s full name printed on it [NO business checks are allowed for anyone being contracted as an individual].
- Starter checks MUST have the agent’s information listed.
BANK LETTER
- MUST be typed.
- MUST have bank letterhead with respective logo(s).
- The account holder’s full name MUST be in the letter.
- Bank account and routing numbers MUST also be included.
DIRECT DEPOSIT FORM
- MUST be pre-printed [Nothing Handwritten]
- The account holder’s information MUST be on the form.
- Bank account and routing numbers MUST also be included.
- MUST clearly state file type [Direct Deposit Form].
- NO pre-paid bank accounts allowed.
